FAQs About Sports Medicine

Q?

Do you accept insurance?

A.

In order to provide high quality care in a relaxed environment, where you and your physician are not rushed, our practice does not bill your insurance company. If you chose, you may apply to your insurance company for reimbursement. We are officially recognized by all major insurance companies and we will provide you with the relevant billing documentation and necessary forms to facilitate this process for you. Our clinic accepts direct payment for services provided at the time those services are rendered. For your convenience we accept cash, Visa, Mastercard, American Express and Discover. We regret that we are unable to accept personal checks. Please see our Financial Policy for more details.

Q?

What are your hours?

A.

We are open weekday mornings by appointment. Meeting times outside of these hours may be available as Dr. Preiss-Farzanegan’s schedule allows (she works around the children’s school hours). We believe your time is valuable and we will do our best to accommodate you and your busy schedule!

Q?

How do I know if I need to see a Sports Medicine Specialist?

A.

If you are an athlete (a high youth, high school, collegiate or professional athlete or an individual who exercises for health or recreation) and you have an issue that is slowing you down or impairing your performance or ability to be active, then a Sports Medicine Specialist can help! Q?

What should I bring to my appointment?

A.

Prior to your appointment, we will email you several forms that we ask you to print, complete and bring with you to your appointment. Please wear comfortable clothes and wear or bring shorts.
