How do I know if I need a sports medicine physician?

Does El Dorado Hills Sports Medicine accept my insurance?

As of 1/1/18, Dr. Preiss-Farzanegan is an approved network provider for the following insurance plans: Aetna, Anthem Blue Cross, Blue Shield, Cigna, Hills Physicians, Humana, Medicare, MultiPlan, Network By Design, United Healthcare and Western Health Advantage. Call the member services number on your insurance card to ask if Dr. Preiss-Farzanegan is an in-network specialist for your insurance plan.

Will I need to pay a co-pay?

A.

If your insurance plan requires a co-payment for a specialist office visit, we will collect it at the time of service. The remainder of fees due will be billed to your insurance company. Patients are responsible for any fees not covered by their insurance.power athletes to achieve their goals.

Do I need a referral to see Dr. Preiss-Farzanegan?

A.

Some insurance plans will require you to obtain a referral from your primary care physician prior to making an appointment at El Dorado Hills Sports Medicine. Call the member services number on your insurance card to find out if you will need a referral prior to making an appointment.

What are your hours?

A.

We are open Monday, Tuesday, Thursday and Friday from 9:00am to 2:30pm, and Wednesday from 10:30am to 2:30pm. Appointments outside of these hours are available upon request and as Dr. Preiss-Farzanegan's schedule allows. We believe your time is valuable and we will do our best to accommodate your busy schedule!

What should I bring to my appointment?

A.

New patients are asked to complete several forms and bring them to their first appointment. Please bring or wear comfortable workout attire (i.e., shorts and t-shirt or tank top/sports bra for ladies) to allow Dr. Preiss-Farzanegan to fully examine the affected area. If you are a runner, please bring your running shoes as well.
